Each city was structured to be a sprawling "where's waldo" of vignettes, with daytime activities showcased on the left and a cutaway for the hospital on the right at night time. 

Every vignette showcases a child or two in some fun activity, along with friends and family. Then those same characters are shown in the hospital setting along with hospital staff. After a couple pages those characters add up.

The situations that the characters find themselves in needed to be shown in small capsules of time that could be translated to the hospital scenes easily. The author gave minimum three specific scenes to be featured per spread.

Since the book centers around a traveling nurse, creating a winding road map for the cover was a natural choice, but it also continues on the interior pages, showcasing the cities as islands along the way.
